I liked the slow buildup to the end--it was a good journey to them putting all their tension and fear to rest. I also liked that both of them were aware of their feelings pretty much all along and chose not to act on them until the time became right for both of them. That made it feel more them, and more real.
There were several points in the story that really moved me, Dean falling into Sam's bed and finding the little car under his pillow being one, but that image of little Sam all alone in the rain and Dean having to watch him was a real heartbreaker, so nicely done! It ties in to the idea that Dean is just completely incapable of not looking out for his brother, right?
